Here’s how it works:
Contact your VA primary care doctor in person or or via My HealtheVet using the following script:
“I am requesting a Community Care referral for acupuncture to treat my [condition]. I would like the referral assigned to David Blatt, L.Ac at Evolution Wellness, a TriWest VA Community Care provider, NPI #1508206731. Please place a Community Care acupuncture consult rather than an internal VA acupuncture referral, as Evolution Wellness is able to see me sooner, and the location is substantially closer to my home. Receiving care there would reduce my travel burden and improve my ability to complete the full course of treatments. My pain significantly affects my daily activities and quality of life, and I'd like to begin treatment as soon as possible.”
It’s helpful to include:
-The condition or symptoms being treated
-How long the problem has been present
-Treatments already attempted, when relevant
Once your authorization is processed (usually 2-4 weeks), you can begin treatments.
Appointments are typically once or twice a week and span 2-3 months, with extensions possible.
